At Canungra Town Medical Centre, our GPs support patients managing chronic conditions such as diabetes, heart disease, asthma, and more, with regular reviews, coordinated care, and practical support.

A key part of this is a structured care plan, which can help you stay on top of your condition and, where eligible, access subsidised allied health services through Medicare.

What Is Chronic Disease Management?

A chronic disease is a health condition that lasts six months or longer, or is ongoing. Managing it well usually involves regular monitoring, adjusting treatment as needed, supporting healthy lifestyle changes, and coordinating care across your GP and other health professionals.

The goal is to help you manage your condition day to day, reduce the risk of complications, and maintain your quality of life. Your GP works with you over time, getting to know your situation and adjusting your care as things change.

Conditions We Help Manage

Our GPs provide ongoing care and support for a range of chronic conditions, including:

  • Type 2 diabetes and pre-diabetes
  • High blood pressure (hypertension)
  • High cholesterol
  • Heart disease and cardiovascular conditions
  • Asthma and chronic obstructive pulmonary disease (COPD)
  • Arthritis and osteoarthritis
  • Chronic kidney disease
  • Thyroid conditions
  • Ongoing mental health conditions (in coordination with our mental health support)

GP Management Plans and Team Care Arrangements

If you have a chronic or ongoing condition, you may be eligible for a structured care plan through Medicare. These help organise your care and can give you access to subsidised allied health support.

GP Management Plan (GPMP) A plan your GP prepares with you that sets out your health needs, goals, and the actions to manage your condition. It’s reviewed regularly, usually every few months, to check your progress and adjust as needed.

Team Care Arrangement (TCA) If your care involves other health professionals, such as a dietitian, physiotherapist, podiatrist, or psychologist, a Team Care Arrangement coordinates that team. With a TCA in place, eligible patients can access a number of subsidised allied health visits each year through Medicare.

Why Regular Reviews Matter

Chronic conditions can change over time, and regular reviews help catch issues early, keep your treatment working well, and keep you involved in decisions about your care. We can send reminders when your reviews or checks are due, so nothing falls through the cracks.

Managing a chronic condition often works best as a team effort. Our GPs coordinate with our dietitian, allied health team, and other providers so your care is joined up rather than fragmented. You don’t need a referral to see your GP about managing a chronic condition. If a care plan and allied health referrals are appropriate, your GP arranges these for you.
