The 75+ Health Assessment

If you’re 75 or older, you’re eligible for an annual Health Assessment with your GP, funded through Medicare. It’s a thorough review of your overall health that looks at your medical conditions, medications, mobility, memory, mood, nutrition, continence, and how you’re managing day to day at home.

The aim is to pick up any issues early, keep you well, and put the right supports in place to help you stay independent. It’s also a good opportunity to talk through any concerns you or your family may have.
